I hereby give Glad Tidings Community Church consent to verify the information provided I have provided in this Volunteer Application and to contact the references listed above, and to obtain and verify any information from them (and any other persons that the Church determines might be able to provide information relevant to this application.

I understand that the Church reserves the right, at any time and at its discretion, to determine that I am not suitable for volunteer service in the Church for the position for which I am applying, and that such termination may come without advance notice.

During the screening process, I agree to provide all documentation required for the Plan to Protect process within six (6) months of my initial application. In the event that I do not provide this information, I understand that I will be removed from the volunteer roster.

If at any time I determine that for any reason I am unable to support or adhere to or follow the policies, procedures, or doctrine of Glad Tidings Community Church, I will inform the Church and will resign my volunteer position(s).
